MEMBERSHIP UPDATE FORM
It is recommended to access and fill this form out using a laptop or desktop device which will allow the form to be filled out digitally. The form may also be printed, filled out by hand, and scanned. The completed form should be emailed as a PDF file attachment to your AV CERT captain.
IMPORTANT: THE COMPLETED FORM MUST BE SAVED AS A PDF FILE USING THE DOWNLOAD BUTTON AND THEN SENT AS AN EMAIL ATTACHMENT TO YOUR AV CERT CAPTAIN.
